What documentation do I need to bring with me to the enrollment meeting?
At initial enrollment meetings you must bring back all the forms given to you by the Enrollment Specialist. It is a good idea to have these forms completed before your appointment to save yourself time during the meeting. You will also need to bring your child’s most recent physical and records for up to date immunizations. For parents utilizing subsidized child care slots, you will also need to bring with you proof of address, proof of income, and the completed employment verification form.
